Completion of Form for PPH

Hi

I received an email from PPH regarding my application.

I was requested to the following:

''Provide your real name.''

 

I have tried to go and edit my name on my Profile.  But i am unable to do so as the sign for 'not allowed' shows up.

 

Please advice on what to do.
