Withdrawal from your PPH Account (Rejected)

Note: Return of original statement entry #11960. PAYOUT RETURNED: Invalid account details.FARRUKH JAVED

Its second time my withdrawal request is rejected, reference the reason above. 

I use this account for withdrawal twice before and MY account status is approved. If you need any details please let me know. 

Please resolve this issue or let me know what details PPH required.

When I submit withdrawal request FIRST TIME my account balance is 578.27 USD (after PPH Fee and Bank transfer fee ) after that it was 515.27 USD again they charge bank transfer fee.

Moreover after third withdrawal request My Account balance is 452.27 USD. Before it was 485.28 USD. I have confirmation emails form PPH,

please resolve this issue and  actual account balance so that I withdraw my funds.
