PPH Approval

I already received a PPH approval, afterwards, they said that I need to edit something which pertains to my real name and location. All details where updated from the start and provided others needed. After 2 submissions PPH rejected due to incompleteness of the details needed even though I already updated the file.
Please help on this. I'm not sure what was the problem.

  my approval has been rejected at 3 times
