Problems verifying my bank & Paypal

My bank account was not approved but I'm not sure what was wrong with the screenshot/details I provided? 

 

My Paypal is still pending also so at the moment I can't withdraw any of my earned funds. Please help!

  Can you help, I have requested the email to verify my account several times and it hasn't come through.
