Where to put name and location?

I received a message telling me to provide my real name and location in my application. When I clicked on the link in the email it took me to the "Tell Us About Yourself" section of my application.

For the life of me, I can't see anywhere that I'm supposed to type in my real name or location. 

Can someone point that out for me? Or tell me how to open a ticket before I have an approved account?
