my application has been rejected

Hi, I am Yash Khatri, I had been submitted application .but I became to know that my application has been rejected the third time, why it happens exactly I can't understand..all the processes I had been done very carefully and correctly also. After that Why my application has been rejected. I want to know the reason. Help me to find where I have done a mistake. please help me.....
