Rejected applications form

Hello sir/ mam                               I'm seller on peopleperhour Account. My problem is that my form is rejected again and again.I am filling all details correctly but I do not  understand where mistake is happening. Why is being rejected. Sir please help me in this . Help me as soon as possible.i will be grateful to you.
