Withdrawal from your PPH Account (Rejected)

Note: Return of original statement entry #11863. PAYOUT RETURNED: Other MS01 NO REASON.FARRUKH JAVED

My bank account is approved so why payment is rejected. 

 

would you like tell inform me about this issue and its solution, Moreover I paid international transfer twice,

when I submit withdrawal request my account balance is 578.27 USD (after PPH FEE and Bank transfer fee ) Now it is 515.27 USD again they charge bank transfer fee?

I received confirmation emails form PPH, please confirm this issue and account balance as well.
