Name and location not accepted, help

I have submitted my application and for some reason they don't accept my name and location and ask for me to provide real ones?

 

Would be grateful if someoje could help me solve the issue.

  After submitting my application again (did not change anything because it was all correct in the first place) this time they said my name is "correct" but still asked to provide my location...which I didn't change because it is correct...so I submitted the same thing again and they have rejected the application...can someone help me sort this nonsense out?
