"Provide real name"

My application came back saying it's incomplete and I need to provide my real name. However there is nowhere in the application process to do this. My real name is in my profile but the boxes are greyed out. How and where do I provide my real name so I can get my application approved?

  We have now changed the name on your account as per your request and notified our moderations team.

  We're now glad to confirm your application has been approved by our moderations team. You can now send proposals for the Jobs and post hourlies to sell your services on the site. I hope your issue is resolved.

  I also have the same problem.  This is in February 2020 and it seems that the problem first occurred 3 years ago.  And it apparently hasn't been fixed since then.  I just spent 2 hours to complete the application and now I can't continue.  Very frustrating.  Also, the Submit a Request button does not work.

  similar problem here, I provided my real name, and still got a request to provide my real name with no actual way to do anything about it...
