Need help verifying my identity

The submit button does not work when trying to upload document.

Also my bank statement is in my married name and I haven't updated my ID or passport yet (they are still in maiden name) I fear this will also be an issue. I can send a copy of my marriage license if this would help.

I would just like access to the money I have earn't, please help
