Can't Withdraw money (no pay-out route)

I can't withdraw my money it's already september, i finished my project and had the money on my pph account last month august. My documents are complete i have a bank and paypal to transfer the money. i really need the money but they rejected me twice and deducted 6 dollars for the 2 reversal fee! i don't understand.

  Fraud by PPH. They have rejected my withdrawal request 4 times and don't provide me any transaction ref number as well.
